What does Tricare require on a prescription?Updated 2 years ago

Tricare has specific requirements for what needs to be written on your prescription (RX). If your RX is missing any of the following, we will request that you obtain a new one or have the missing items added to your current RX.

  • Mom's Full Name and Date of Birth
  • Sponsor’s SSN and/or Patient DoD Benefits Number
  • Patient Phone
  • Baby's Due Date OR Date of Birth (If Baby Born)* (Tricare West only)
  • Doctor's Signature and Doctor's NPI
  • Must be signed by valid prescriber (not LPN, RN, or IBCLC)
  • Provider Phone Number
  • Doctor's Date Prescribed
  • E0603 Electric breast pump purchase
  • Diagnosis (Tricare East only)
  • Lactating Mother Z39.1, or other
  • # of Weeks Pregnant, Due Date or Age of Infant (Tricare West requires that # of weeks pregnant be 27 or greater)
