No, upgrade fees are not covered by your plan. The insurance will only pay the single contract rate, which covers only your basic choices. The upgrade fee is a completely voluntary choice made by you to receive a deluxe pump model, and pay the difference. Every plan is offered at least one fully covered option as required by the ACA.
